PREPARATION INSTRUCTIONS FOR CLINICAL LAB TESTS

Dear Client;

The importance of true test results depends upon following these instructions carefully:

1. Follow normal or usual diet the day before test is to be taken.

2. DO NOT take any vitamins, minerals, or supplements of any kind or

any type of medication except prescribed medication such as

Digitalis (cardio-tonics), Insulin, etc., the day before the test is

to be taken.

3. The importance of a 14 to 16 hour fast is essential for a base line.

For example, if you have a 9:00am appointment, eat and drink nothing

after 6:00pm the evening before. You may have WATER ONLY.

4. NaturChem needs one (1) urine specimen. The first specimen in the

morning is best. Pour Specimen into a sterile container or yellow

top container with tablet. Take to the lab on the day of the test.

5. Take the General Test Requisition Form with marked profile ordered

plus the morning urine specimen to the lab of your choice.

6. NaturChem will call and make an appointment with the lab of your

choice for a time and date, thus avoiding a long wait.
