PREPARATION INSTRUCTIONS FOR Dr. Al Aguirre’s CLINICAL LAB TESTS

Dear Client;

The importance of true test results depends upon following these instructions carefully:

 

1. Follow normal or usual diet several days before test is to be taken.

2. DO NOT take any vitamins, minerals, or supplements of any kind or

   any type of medication except necessary prescribed medication such

   as Digitalis (cardio-tonics), Insulin, etc., the day before the test

   is to be taken.

3. The importance of a 12 hour fast is essential for a base line. 

   For example, if you have a 9:00am appointment, eat and drink nothing

   after 6:00pm the evening before. You may have WATER ONLY w/lemon, if

   necessay vegetabe jucie is okay.

4. Our service will call and make an appointment with the lab that we

   work with, with the time and date you specify-avoiding a long wait.

5. Our service will Fax a requisition form directly to a lab we

   work with, which is closest to your (zip-code).

Special Test Screen Ordered is NCHS2 (NaturChem Health Screen)

Items Needed:  2-Glass Blood slides.

               2-15ml. Red top serum separator tubes. Invert at least

                       six times and then let stand 30 minutes,

                       centrifuge 15 minutes and do not pour into serum

                       vial, need two 10 or 15ml separator tubes

                       unopened for CO2 test accuracy.

               1-5ml. Lavender top, which has been inverted at                     

                 least 6 times. (2-Glass Blood slides)

               1-Requisition Form filled out.
